So, let's get started. First, for your safety and well-being, our doctors have requested that each "NEW" patient follow these guidelines:

  1. GO online or give us a call and speak to one of our Patient Care Managers and complete our medical history and assessment (5 minutes).
  2. If requesting medications—FAX a copy of your valid photo ID and your most recent medical records or fill out the Medical Records Release form to allow us to obtain the records for you. (Records must be within the last 12-24 months). If you are requesting a refill prescription of medications, the doctors require a history and physical once a year.  Click here to download the required forms.  If you do not have these items, a doctor or Patient Care Manager will discuss your options during pre initial consult.
    Please note: Your records should clearly document the condition for which you are requesting a consultation.

  3. Have a PHONE consultation with one of the Telemedicine Physicians to discuss your case and treatment options.

Please note: If after reviewing your case the doctor determines that your condition requires medication, you will be given a valid prescription.

We also provide e-health technology that allows doctors to interact with their patients regardless of distance or remote locations by using a combination of phone, fax, Internet and/or home health monitoring equipment and solutions otherwise known as telehealth or telemonitoring.

Telehealth uses the Internet and telecommunications devices including telephones, fax machines, at-home exams, web based tools, home health monitoring equipment and solutions placed in patients’ homes to take their vital signs every day, remind the patient to take medication and answer simple questions that help the doctor know any change in the patient’s condition.
